About the Role
Avalon Rehabilitation and Healthcare Center is seeking a licensed Social Worker to provide emotional, social, and psychosocial support to residents, helping them achieve the highest possible quality of life. This role is proudly supported by Marquis Health Consulting Services and operates within a 180-bed Sub-Acute and Long-Term Care facility designed with beautiful common spaces for a welcoming, home-like environment.
Rate Range: $25/hr. - $40/hr. (all-inclusive based on experience). Must have experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting.
Responsibilities
- Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations.
- Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
- Build and maintain positive, supportive relationships with residents and their families.
- Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
- Develop, implement, and update individualized psychosocial care plans for residents.
- Participate in care planning meetings and support continuity of care throughout the resident’s stay.
- Provide counseling, emotional support, and resource referrals as needed.
- Ensure compliance with all applicable facility policies, regulations, and documentation standards.
Requirements
- Must be able to work full-time hours and maintain a flexible schedule.
- Bachelor’s degree required; Master’s degree strongly preferred.
- Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting; experience in a nursing home or rehabilitation center strongly preferred.
- Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
- Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
- Ability to work effectively in a fast-paced, team-oriented healthcare environment.
