Social Worker

About the role

As a Social Worker at DaVita, you’ll be a vital member of each patient’s core care team in an outpatient dialysis setting. You will support and advocate for patients, helping them understand their rights and responsibilities while guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. This role focuses on building meaningful, long-term relationships with patients and their families.

Responsibilities

• Support patients in navigating the challenges of dialysis and End Stage Renal Disease.
• Advocate for patients’ rights and help them manage their healthcare journey.
• Guide patients and families in addressing the mental, emotional, and financial aspects of their condition.
• Collaborate with a clinical team in a positive, supportive environment.
• Participate in clinical goal-setting and quality improvement initiatives.

Requirements

• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Functional proficiency with DaVita-specific applications within 60 days.
