Responsibilities

• Support and advocate for patients undergoing life on dialysis.
• Help patients understand their rights and responsibilities related to End Stage Renal Disease.
• Guide patients in managing the physical, mental, emotional, and financial demands of their condition.

Requirements

• Master's degree in Social Work (MSW) required with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Mandarin/Cantonese speaking highly preferred.

Qualifications

• Meets all state required regulations to practice in the dialysis setting.
• Functional proficiency with DaVita specific applications within 60 days.

Skills

• Strong interpersonal and communication skills.
• Ability to build and maintain strong relationships with patients and their families.
• Knowledge of end-stage renal disease and its management.

Pay

The Salary Range for the role is $75,000 - $118,000/year. Social Worker Intermediate: $81,000 - $88,000/year.
