Social Worker - Adolescent Partial Hospitalization Program

About the role

The Adolescent Partial Hospitalization Program (PHP) at Love + Medicine is seeking a licensed social worker to co-facilitate and facilitate therapeutic groups and provide direct support to adolescents ages 13–18 during program hours (9:00 a.m.–3:00 p.m.).

Responsibilities

• Co-facilitate and facilitate therapeutic groups
• Provide direct support to adolescents ages 13–18 during program hours (9:00 a.m.–3:00 p.m.)
• Work closely with youth in a structured, team-based setting to support emotional regulation, skill development, and overall treatment engagement throughout the day
• Collaborate with the treatment team to coordinate patient care, provide or assist with crisis intervention, meet with patients, and assist with programming needs
• Act as a consultant and leader in comprehensive care throughout the system, while supporting organizational strategies and goals

Requirements

• Bachelor’s degree in Social Work or in a related field
• WI Social Worker license
• 1 year of experience in human services or health care
