Responsibilities

• Provide hospice care services to patients in their homes or at designated facilities.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Assist with end-of-life care, including symptom management and emotional support.
• Maintain accurate records and documentation according to regulatory standards.

Requirements

• Bachelor's degree in a healthcare-related field.
• Current certification in Hospice and Palliative Care (CHPC).
• Experience in hospice or palliative care preferred.
• Strong communication and interpersonal skills.

Qualifications

• Valid driver’s license and reliable transportation.
• Ability to work flexible hours, including evenings and weekends.

Skills

• Excellent communication skills.
• Strong organizational and time management skills.
• Proficiency in Microsoft Office Suite.
