About the role
The Social Worker / Admissions Coordinator provides social work services for applicants and residents of the Minnesota Veterans Home. Responsibilities include assessing and determining proper placement of applicants requesting admission, offering clinical support to address residents' needs, and planning and implementing social service programs. This position works closely with the Home Administrator to ensure regulatory compliance and high standards in social services delivery.
Responsibilities
- Conduct social work assessments to evaluate clients’ social service needs.
- Provide input to determine proper placement of applicants requesting admission.
- Offer clinical support and analysis to ensure residents' needs are addressed.
- Plan and implement social service programs for residents.
- Collaborate with the Home Administrator to ensure social services meet regulatory requirements and agency standards.
- Develop and maintain effective working relationships with staff, residents, resident representatives, and community groups.
Requirements
- Current licensure in Minnesota as a Licensed Social Worker (LSW) or ability to obtain licensure by date of hire.
- One year of Social Work experience.
- Successful completion of a Minnesota Department of Human Services (DHS) background check (conducted on top finalist(s)).
- Tuberculosis (Mantoux) screening or chest x-ray required prior to employment.
- Employment reference checks will be conducted on all finalists.
Preferred Qualifications
- Experience coordinating placements in senior housing or long-term care services.
- Experience in a long-term care or skilled nursing care facility.
- Experience working with Veterans.
- Knowledge of federal and state regulations related to skilled nursing facilities.
- Experience with electronic charting systems such as PointClickCare.
Skills
- Strong skills in completing social work assessments.
- Excellent human relations skills to foster effective working relationships.
Pay
$26.12 - $37.91 / hourly; $54,538 - $79,156 / annually
