Location: Fergus Falls
Full-time, unlimited position on the day shift (Monday - Friday).
About the role
The purpose of the position is to provide social work services for the applicants and residents of the Minnesota Veterans Home, to provide assessment and input to determine proper placement of applicants requesting admission, to provide clinical support and analysis to assure residents' needs are addressed, and to carry out planning and implementation of social service programs for residents of the Home. This position will also work effectively with the Home Administrator to ensure the delivery of all social services meets regulatory requirements and agency standards.
Responsibilities
- Provide social work services for applicants and residents of the Minnesota Veterans Home.
- Assess and determine proper placement of applicants requesting admission.
- Provide clinical support and analysis to address residents' needs.
- Plan and implement social service programs for residents.
- Ensure delivery of social services meets regulatory requirements and agency standards.
Requirements
- Current licensure in Minnesota as a Licensed Social Worker (LSW) or ability to obtain licensure by date of hire.
- One year of Social Work experience.
- Successful completion of a Minnesota Department of Human Services (DHS) background check (conducted on top finalist(s)).
- Tuberculosis (Mantoux) screening (or chest x-ray) prior to employment.
- Employment reference checks for all finalists.
Qualifications
- Skills in completing social work assessments sufficient to evaluate clients’ social service needs.
- Human relations skills sufficient to develop and maintain effective working relationships with staff, residents, resident representatives, and community groups.
Preferred Qualifications
- Experience coordinating placements in senior housing or long-term care services.
- Experience in a long-term care or skilled nursing care facility.
- Experience working with Veterans.
- Knowledge of federal and state regulations related to skilled nursing facilities.
- Experience in electronic charting systems such as PointClickCare.
Pay
$26.12 - $37.91 / hourly; $54,538 - $79,156 / annually
