Responsibilities
- Serves as the case manager for assigned patients and coordinates the interdisciplinary assessment process.
- Completes psychosocial assessments within organizational guidelines.
- Conducts group and individual therapy and/or case management activities as appropriate to the treatment plan.
- Conducts family meetings as necessary and as directed by the physician.
- Develops and implements high-quality patient treatment plans based on thorough patient assessment, family and home dynamics, and patient’s capacity to cope with daily living problems.
- Follows established practice guidelines for work and patient scheduling, demonstrating flexibility to meet practice and patient needs.
- Provides 24-hour on-call coverage for the center on a rotating basis and manages crisis issues.
- Reports cases of suspected abuse or neglect while maintaining patient and family confidentiality.
- Completes treatment plan documentation and updates according to patient needs and organizational expectations.
- Documents all patient and caregiver interactions, group, individual, and family activities.
- Explains living wills and durable power of attorney to patients and family members in a sensitive manner.
- Follows policies and procedures for discharge planning, evaluations, chart documentation, and follow-up coordination.
- Completes billing forms, writes letters and reports, and ensures timely completion of authorizations.
- Develops instructional materials for cardiac rehabilitation patients to facilitate recovery and promote wellness.
- Works collaboratively to solve problems, improve processes, and develop services as a patient advocate.
- Develops cooperative agreements with community agencies to meet patient needs.
- Participates in departmental and organizational quality improvement initiatives.
- Provides counseling and facilitates staff support groups to promote a positive working environment.
- Actively participates in the development and implementation of interdisciplinary treatment plans and team activities.
- Promotes a therapeutic milieu through formal and informal methods.
- Acts as a clinical resource and mentor to healthcare team members, displaying leadership behaviors.
- Identifies learning needs based on self-evaluation and progress from previous goals.
- Completes all required continuing education and competencies relevant to the area of practice.
- Ensures improvements in practice settings through lifelong learning and self-development.
- Provides direction, guidance, and mentorship to others regarding practice.
- Demonstrates leadership skills in critical thinking, decision-making, and problem-solving.
- Participates in community outreach services as an educational resource about care management programs.
- Provides professional consultation and in-service education to medical and hospital staff.
- Performs other duties as assigned or requested.
Qualifications
- Some previous experience in social work.
- Master’s Degree in Social Work or related field.
- Crisis Intervention Training within 90 days of employment.
- Proficient in English language, verbal and written communication skills, and basic computer skills.
Licensure
- Licensed Clinical Social Worker (LCSW).
- Basic Life Support (BLS) Certification.
