Social Worker 3 - LCSW - Angels Family Medicine Clinic

Responsibilities

• Serves as the case manager for assigned patients.
• Counsels patients and family members in a sensitive and caring manner.
• Coordinates the interdisciplinary assessment process.
• Completes psychosocial assessments and develops and implements patient treatment plans.
• Participates in treatment team conferences.
• Provides 24-hour on-call coverage and manages crisis issues.
• Documents patient interactions, activities, and treatment plans.
• Develops and explains instructional materials for cardiac rehabilitation patients.
• Collaborates with community agencies to meet patient needs.
• Facilitates staff support groups and promotes a positive working environment.
• Assists in the development and implementation of the interdisciplinary treatment plan.
• Provides high-quality professional consultation and in-service education to medical and hospital staff.

Requirements

• Master's Degree in Social Work.
• Licensed Clinical Social Worker (LCSW) license in Louisiana.
• Crisis Intervention Training within 90 days of employment.
• Experience in social work.
• Basic computer skills.
• Proficient in the English language, verbal and written communications skills.

Qualifications

• Some previous experience in social work.
• Current Louisiana State Board of Social Workers License.
