Social Worker 3 - LCSW

Responsibilities

• Serves as the case manager for assigned patients.
• Collaborates with the interdisciplinary team to coordinate the assessment process.
• Completes psychosocial assessments and conducts group and individual therapy as needed.
• Develops and implements patient treatment plans based on thorough assessments and collaboration with the treatment team.
• Manages crisis situations and follows on-call schedule as required.
• Documents patient interactions, activities, and progress in accordance with organizational standards.
• Provides counseling and support to patients and their families, including explaining legal documents like living wills and durable powers of attorney.
• Participates in discharge planning and follows up with patients as necessary.
• Facilitates staff support groups and promotes a positive work environment.
• Assists in the development and implementation of the interdisciplinary treatment plan.
• Engages in professional development through self-assessment and participation in continuing education.

Qualifications

• Master's Degree in Social Work.
• Current Louisiana State Board of Social Workers License.
• Completed Crisis Intervention Training within 90 days of employment.
• Some previous experience in social work.
• Proficiency in English, both verbally and in writing, and basic computer skills.
