About the role
This position is open to current Colorado residents only. It is a full-time, Monday-Friday position at CMHHIP, providing comprehensive psychiatric and psycho-social evaluation and treatment under the supervision of an LCSW.
Responsibilities
- Completes psychosocial assessments by interviewing, collaborating, analyzing, and assimilating social histories demonstrating the relationship among current behavior problems, environmental resources, and support systems.
- Generates clinical summaries and treatment recommendations that predictably affect treatment and continuity of care in the community.
- Coordinates discharge planning, evaluates patient needs, and explores and mobilizes community resources.
- Identifies and chooses appropriate community placements and treatment programs, and provides case management by making referrals to community agencies.
- Attends meetings with various agencies to ensure continued appropriate level services following discharge.
- Participates in the direction and formulation of treatment plans and reviews, collaborates with multidisciplinary treatment teams, and recommends and designs treatment interventions.
- Provides individual, family, and group therapy and system interventions utilizing social work theoretical frameworks and methods, considering ethnic, cultural, religious, and sexual diversity.
- Keeps updated to the state of the art theories for assessment and treatment of psychiatrically impaired patients, and as appropriate, for patients dually diagnosed with dementia, developmental disabilities, or substance-related disorders.
- Works with other team members to gather appropriate outcome data to assess the effectiveness of treatment.
- Performs other duties as assigned.
Requirements
Minimum Qualifications:
- Four (4) years of relevant experience in an occupation related to the work assigned to this position OR a combination of related education and/or relevant experience in an occupation related to the work assigned equal to four (4) years.
- A Master's degree in Social Work (MSW).
Preferred Qualifications
- Experience in casework methods, techniques, and their applications to work problems.
- Knowledge of principles of human growth and behavior, basic sociological and psychological treatment and therapy practices.
- Interviewing skills and techniques.
- Effective oral and written communication skills.
- Ability to exercise good judgment in evaluating situations and making decisions.
Conditions of Employment

- State of Colorado residency at the time of application and throughout the duration of employment.
- Valid, non-restricted Colorado Driver’s License or equivalent.
- Pass motor vehicle check.
- Pass out-of-state driver’s license requirement within 30 days of employment start date.
- Pass 2-step TB screening upon hire.
- Annual influenza vaccination.
- Successfully complete CMHIP New Employee Orientation core curriculum.
- Pass all modules of the CMHIP New Employee Orientation core curriculum to include English reading comprehension, relevant competency assessments, and written/practical exams in Defensive Tactics.
- Physical ability to complete hospital required physical intervention training, MANDT, and CPRL.
- Ability to see, hear, and work in environments with fluctuating noise and temperature, including working outdoors in inclement weather and/or concrete floor/wall work settings.
- Willingness and ability to travel.
