Social Work Case Manager

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
• Affirms patients' and families' wholistic needs for discharge planning, incorporating clinical, social, and financial factors into the transition of care plan.
• Develops discharge plans with contingency plans, ensuring timely care coordination and progression of care, and arranging post-acute care services and facilities.
• Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities for continuity of care, ensuring accurate discharge reconciliation.

Requirements

• Master's degree [Required]
• 2+ years of care management experience [Preferred]
• 2+ years of social work experience [Required]
• Hospital social work experience [Preferred]
• Active State of Illinois LSW or LCSW [Required]
• Accredited Case Manager (ACM) [Preferred]
• Certified Case Manager (CCM) [Preferred]

Qualifications

Education: Master's [Required]

Experience: 2+ years of care management experience [Preferred], 2+ years of social work experience [Required], Hospital social work experience [Preferred]

Skills

Technology proficiency, communication skills, empathy, organizational skills, and ability to manage complex cases.

Pay

$25.65 - $52.47

Schedule

Full time, Day shift, Rotating weekends and holidays as needed
