About the role
The Social Work Care Manager I- IPCM provides comprehensive care coordination and support services to individuals and families facing complex social and health-related challenges. This role works collaboratively with clients, their families, healthcare professionals, and community resources to address their unique needs and improve their overall well-being.
Responsibilities
- Conduct thorough assessments of clients' social, emotional, and environmental circumstances
- Develop personalized care plans
- Congratulate access to necessary services and resources
- Advocate for clients' rights and empower them
- Engage in crisis intervention, counseling, and advocacy
- Promote preventive measures and education within the community
Requirements
- Masters in Social Work, or has satisfactorily completed all requirements for the MSW but awaiting conferment by the educational facility, for all Social Worker’s hired after 12/25/2011.
- No experience required.
- Current license in Social Work in the state of employment (LMSW, LCSW).
- BLS (Basic Life Support) (CPR) at hire date, required, or within 90 days (about 3 months) of hire.
- Certification in Case Management preferred.
- 2 years’ experience in acute care hospital setting preferred.
