About the role
The Care Manager, as part of the interdisciplinary team, assesses, plans, advocates, and coordinates care from admission to discharge ensuring a safe transition post hospitalization. Duties and responsibilities are performed in a manner consistent with the Mercy mission, values, and service standards.
Responsibilities
- Affords effective transition planning and length of stay oversight to maintain patient experience, safety, and quality of care utilizing performance metrics and adoption of best practices.
Requirements
- Master's in Social Work, or has satisfactorily completed all requirements for the MSW but awaiting conferment by the educational facility, for all Social Workers hired after 12/25/2011.
- 0-2 years' experience in acute care hospital setting, Minimum Preferred.
- Current license in Social Work in the state of employment (LMSW, LCSW).
- BLS (CPR) at hire date, required, or within 90 days of hire.
- Certification in Case Management, Preferred.
