Social Work Care Manager - Full-Time

This role involves providing telephonic support to members' medical, psychosocial, physical, and spiritual needs. It includes developing, implementing, and monitoring person-centered service plans to ensure continuity of care across healthcare settings through collaboration with interdisciplinary teams, primary care providers, members, and specialists. The position requires utilizing benefit structures and community resources to meet members' needs, educating them on disease management, preventive health, and quality of life. Knowledge of CMS/DOH regulations and policies is essential. Qualifications: Bachelor’s degree in social work or related field; MSW or LMSW preferred.Experience: Minimum three to five years in clinical social work within long-term care, managed care, home care, geriatrics, or hospice. Prior interdisciplinary service delivery experience preferred.Knowledge: Familiarity with Medicare and Medicaid regulations preferred.Licensing: Current LMSW in New York State preferred.
