Social Work Care Manager - Full-Time

About the role

This role involves providing telephonic support to members, addressing their medical, psychosocial, physical, and spiritual needs.

Responsibilities

• Developing, implementing, and monitoring person-centered service plans
• Ensuring continuity of care across healthcare settings through collaboration with interdisciplinary teams, primary care providers, members, and specialists
• Utilizing community resources and benefit structures to meet member needs
• Educating members on disease management, preventive health, and quality of life

Qualifications

• Bachelor’s degree in social work or related field; MSW or LMSW preferred
• Minimum three to five years of clinical social work experience in long-term care, managed care, home care, geriatrics, or hospice
• Experience in interdisciplinary service delivery environments is preferred
• Knowledge of CMS/DOH regulations is essential
• Knowledge of Medicare and Medicaid regulations is advantageous
• Current LMSW in New York State is preferred
