Social Work Care Manager - Full-Time

This role involves providing telephonic support to members' medical, psychosocial, physical, and spiritual needs. Responsibilities include developing, implementing, and monitoring person-centered service plans, ensuring care continuity across healthcare settings through collaboration with interdisciplinary teams, primary care providers, members, and specialists. The role requires utilizing community resources and benefit structures to meet members' needs, educating them on disease management, preventive health, and quality of life strategies. Adherence to CMS/DOH regulations and policies is essential. Qualifications And Skills Bachelor’s degree in social work or related field; MSW or LMSW preferred.Minimum of three to five years of clinical social work experience in long-term care, managed care, home care, geriatrics, or hospice.Experience in interdisciplinary service delivery environments preferred.Knowledge of Medicare and Medicaid regulations preferred.Current LMSW credential in the relevant state is preferred.
