Responsibilities

Qualifications & Skills
- Licensed Clinical Social Worker (LCSW) credential.
- Strong assessment, care planning, and coordination skills.
- Experience with EMR systems and outcome tracking.
- Excellent collaboration and communication abilities.
- Patient-centered approach with a focus on holistic care.
