Social Work Care Manager

About the role

The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources and qualify for community assistance from a variety of special assistance programs and agencies. The role assists with transitions of care or discharge planning, offers crisis intervention to patients and families with psychosocial needs, and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This position receives referrals for individuals from at-risk populations from interdisciplinary team members including physicians, RN Care Managers, staff nurses, and others.

Responsibilities

• Intervene with patients who have complex psychosocial needs
• Assist with eligibility determination for social programs, funding sources, and community assistance
• Provide assistance with transitions of care or discharge planning
• Offer crisis intervention to patients and families with psychosocial needs
• Coordinate and facilitate the development of a discharge plan of care for high-risk patient populations
• Receive and process referrals for individuals from at-risk populations from interdisciplinary team members

Education

• Master's degree in Social Work (MSW) required

Work Experience

• 2+ years of social work experience required
• 2+ years of care management experience preferred

Pay

• $23.94 - $44.53 per hour

Schedule

• Full time
• Shift: Days
• Schedule: 5 days/week, 8-hour shifts (Monday–Friday), with 4 individual weekend shift requirements in a 6-week period
