Social Work Care Manager

This role involves providing telephonic support to members' medical, psychosocial, physical, and spiritual needs. It requires developing, implementing, and monitoring person-centered service plans to ensure continuity of care across healthcare settings through collaboration with interdisciplinary teams, primary care providers, members, and specialists. The position includes utilizing benefit structures and community resources to meet member needs, educating members on disease management, prevention, and quality of life improvements, all while adhering to CMS/DOH regulations and policies. Key qualifications include a bachelor’s degree in social work or a related field, with MSW or LMSW preferred. Candidates should have three to five years of clinical social work experience in long-term care, managed care, geriatrics, or hospice, and ideally experience in interdisciplinary service environments. Knowledge of Medicare and Medicaid regulations is advantageous, and current LMSW licensure in New York State is preferred. Skills required include strong communication, care coordination, and regulatory compliance to support member independence and well-being.
