Social Work Associate

Reports to: Director of Social Work – Queens/Manhattan, New York, United States

Responsibilities

• Work with clinical social worker to provide social work services at the service units
• Complete psychosocial assessment for patients as indicated
• Formulate and implement the intervention plan for patients
• Provide concrete case management, follow-up, and referral service
• Provide entitlements/benefits eligibility counseling and assist with application
• Assist Clinical Social Workers in providing case management service as needed
• Ensure timely services provided to patients based on the Social Work department guideline
• Complete documentations and reports based on the Social Work department guideline
• Attend case conference, agency meetings, and staff meetings
• Participate in community outreach and marketing activities
• Perform other duties as assigned

Requirements

• Bachelor's Degree in Social Science, Social Work, Psychology, Sociology, or other related field
• At least one year of working experience in social services, health care agency, or related field
• Good verbal and written communication skills
• Bilingual in English/Chinese (Mandarin/Cantonese) required
