Social Worker Associate

Purpose Statement/Position Summary

This position will support the NATEEN Teen Parent Support Program at CHLA, providing case management services to expectant and parenting youth, conducting home visits, linking youth to community partners, assisting with school enrollment, health education, and other resources. Under the supervision of a Clinical Social Worker, responsibilities include comprehensive transitional case management, follow-up on referrals, data entry, and collaboration with other Care Services team members to enroll youth in appropriate services. The Social Worker Associate will also develop relationships with community-based agencies and provide technical assistance and training.

Required Minimum Qualifications/Work Experience

• Valid CA Driver’s License and proof of insurance or access to reliable transportation may be required for off-site duties.
• 2+ years’ experience providing case management or clinical services to At-risk adolescents.
• Experience in providing services to At-risk youth.
• Experience in working with diverse populations.
• Familiarity with positive youth development frameworks.
• Knowledge of adolescent development and sexual health.
• Health education experience (contraception, sexual decision-making, and/or family planning).
• Interest in working with adolescent population and capability to build rapport with youth.

Education/Licensure/Certifications

• Bachelor’s Degree required.
• American Heart Association BLS certification required.
