Job Responsibilities

• Leads, mentors, educates and provides guidance to the team
• Works closely with other stakeholders to support the overall care coordination strategy
• Develops strategies and implements best practices and workflows that support BPP contractual arrangements
• Collaborates with community partners and Social Responsibility to connect patients to appropriate resources

Experience Requirements

• 3-5 Years Leadership Experience

Education And Credential Requirements

• Master of Social Work (required)
• Doctorate of Arts/Science (preferred)
