About the role
We are seeking an experienced and compassionate Social Services Director who will be responsible for addressing the social, emotional, and psychological needs of each resident. This role involves helping residents achieve their full potential during their stay at the facility and assisting with their admission/discharge planning.
Responsibilities
- Develop individualized social histories, assessments, and care plans for each resident upon admission, identifying their social, emotional, and psychological needs, and setting realistic goals for their care.
- Participate actively in interdisciplinary Care Plan meetings, ensuring comprehensive and integrated care for all residents.
- Document progress notes and updates regarding each resident's care, ensuring compliance with facility policies and regulatory timeframes.
- Assist in discharge and placement planning, providing guidance and support to residents and their families.
- Organize and facilitate family group sessions to improve communication and provide education and emotional support to family members.
- Educate the community on aging, resident rights, available facility services, and other relevant topics.
- Support the pre-admission screening of potential residents and guide them and their families through the admission process.
- Provide orientation and ensure a smooth adjustment for residents entering the long-term care environment.
- Maintain accurate records and manage the social services budget and supplies, adhering to company policies and state and federal regulations.
Qualifications
- Minimum of 2 years' experience in social work, with a preference for healthcare or long-term care experience.
- Must hold a valid Social Worker license.
- Bachelor's Degree in Social Work (Master's Degree in Social Work, where required).
- Proficiency in computer technology and documentation software.
- Strong communication skills (both written and verbal) and the ability to engage effectively with residents, families, and interdisciplinary teams.
- Knowledge of state and federal regulations related to social services in healthcare.
- Demonstrated ability to work collaboratively with staff, families, and external agencies.
