Social Services Director

What You’ll Do

• Lead and manage the Social Services department in compliance with federal, state, and company guidelines
• Complete social histories, psychosocial assessments, care plans, and required documentation
• Cook discharge planning and community resource referrals
• Support residents and families with transition planning, emotional support, and resident rights advocacy
• Participate in interdisciplinary care plan meetings and quality assurance initiatives
• Assist residents with Advance Directives and psychosocial needs
• Collaborate with hospitals, hospice agencies, outpatient providers, and community organizations
• Maintain accurate documentation within PointClickCare and other systems
• Educate residents, families, and staff on resident rights, abuse prevention, and psychosocial care
• Prepare the department for state survey readiness and compliance

Qualifications

• Required: Licensed Social Worker (LSW) in the State of Indiana
• Preferred: Skilled Nursing Facility (SNF) or Long-Term Care experience
• Preferred: Experience with PointClickCare or electronic medical records
• Preferred: Knowledge of Medicare, Medicaid, PASRR, and state/federal regulations
