Overview
As the Social Services Director, you will identify and address the social, emotional, and psychological needs of each resident, assisting in their development and preparing them for discharge planning.
What You Will Do
- Assist residents and their families with social, emotional, and family issues.
- Provide timely and accurate documentation to comply with Facility, State, and Federal policies.
- Participate in resident care planning and discharge planning.
- Serve as a source of information for residents and their families.
Qualifications
- Bachelor’s or Master’s Degree in Social Work or Psychology.
- 3+ Years Experience working in Social Services, preferably in a healthcare setting.
- High level of professionalism and confidentiality.
