About the Role

We are looking for a compassionate Social Worker with an innate desire to help people. Join our team to improve the services we provide to residents.

Responsibilities

  • Identify psychosocial, mental, and emotional needs of residents and provide, develop, or aid in accessing services to meet those needs in accordance with the National Association of Social Workers (NASW) Code of Ethics and Federal, State, and Local regulations.
  • Complete social history and psychosocial assessment for each resident, including relevant parts of the minimum data set (MDS).
  • Maintain accurate and timely documentation complying with federal/state regulations, corporate policy, and center practices (e.g., Advanced Directives, MDS, Social Service History and Assessment, RAPs, Care Plans, Progress Notes, Behavior Tracking, PASARR, and Discharge Planning).
  • Support residents' right to self-determination; document refusals of services and inform residents of therapeutic alternatives.
  • Participate in developing a written, interdisciplinary care plan for each resident that identifies psychosocial needs, goals, and appropriate interventions.
  • Provide therapeutic interventions to assist residents in coping with transition and adjustment to long-term care, addressing social, emotional, and psychological needs.
  • Offer support and education to residents, family members, and significant others to help them understand placement and facility issues; refer to appropriate social service agencies when needed.
  • Coordinate the resident discharge planning process and make referrals for appropriate home care services.
  • Work with the interdisciplinary team and administration to promote and protect resident rights and psychological well-being.
  • Prevent and address resident abuse as mandated by law and professional licensure.
  • Encourage residents, families, and staff to take a proactive role in self-advocacy to improve quality of life and care.
  • Assist the Social Service Director/Consultant in planning, developing, organizing, implementing, evaluating, and directing social service programs.
  • Work with the Director of Nursing and Administration to prepare for inspection surveys, engage with inspectors, and assist in immediate corrections of noted problems.
  • Review and reinforce important standards previously cited.
  • Perform other duties, responsibilities, and activities as assigned.

Requirements

  • Certified, licensed, or registered in the state of practice (required).
  • Bachelor’s Degree in Social Work or a related Human Services field (e.g., Sociology, Special Education, Rehabilitation Counseling, Psychology) from an accredited school (required).
  • One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
