Social Services Designee-Part time

Position Summary

We are seeking a compassionate and detail-oriented Social Work Assistant / Social Services Designee to support the psychosocial well-being of residents in our skilled nursing facility. This role offers a flexible 3-day-per-week schedule, allowing you to select the days that work best for you.

Key Responsibilities

• Conduct psychosocial assessments and complete social histories
• Participate in MDS assessments, care plans, and interdisciplinary meetings
• Provide emotional support and adjustment counseling for residents and families
• Coordinate discharge planning and community resource referrals
• Maintain accurate documentation (MDS, care plans, progress notes, PASARR, etc.)
• Support resident rights, advocacy, and quality of life initiatives
• Assist with survey readiness and compliance (CMS/state regulations)
• Investigate concerns and support abuse prevention and reporting protocols
• Collaborate with nursing, therapy, and administration teams

Qualifications

• Licensed or certified in Social Work or Human Services (state requirements apply)
• Bachelor’s degree in Social Work, Human Services, Psychology, or related field
• 1+ year of social work experience in healthcare or long-term care
• Skilled nursing facility (SNF/LTC) experience preferred
• Knowledge of MDS, care planning, and CMS regulations strongly preferred
• Strong communication, organization, and documentation skills
