Responsibilities
- Help develop, implement, and maintain social services policies and procedures.
- Assess residents' psychosocial, emotional, and medically related social service needs.
- Support residents and families with adjustment to illness, long-term care, discharge planning, and end-of-life concerns.
- Maintain accurate social service documentation, progress notes, and required reports.
- Provide guidance on Medicare, Medicaid, financial assistance, and available community resources.
- Cook up referrals and services with community agencies and healthcare providers.
- Aid in discharge planning, including arranging equipment, home services, and agency referrals.
- Participate in care planning meetings and interdisciplinary team collaboration.
- Support continuous quality improvement by participating in departmental meetings and policy reviews.
- Build and maintain positive relationships with residents, families, staff, and community partners.
Requirements
- Must possess, as a minimum, a high school diploma or its equivalent.
- Prior experience in Social Services or similar role in a healthcare or post-acute setting preferred.
- Demonstrated commitment to resident-centered care and strong interpersonal skills.
- Excellent organizational skills and the ability to manage documentation and compliance requirements effectively.
- Ability to work effectively with multidisciplinary teams and establish positive relationships with residents and families.

Wage ranges from $20.00 - $30.00 depending on experience.
