About the role
We are currently searching for a Social Services Designee to join our friendly, caring and supportive team at Southfield Wellness Community. In this role, you will assist in preparing our residents for their future by ensuring their social and emotional needs are met during their stay, coordinating resident discharges, and participating in resident care conferences. Your work will help maintain the high standard of care our residents expect, ensuring they are comfortable and safe.
Responsibilities
- Ensure residents' social and emotional needs are met during their stay.
- Coordinate resident discharges.
- Participate in resident care conferences.
Requirements
- Prior Social Services Designee experience in a Skilled Nursing Facility.
- Bachelor’s degree in Social Work or a similar field (preferable).
- Five (5) years of related experience (preferable).
