Social Services Designee
About the role
The Social Services Designee coordinates the overall interdisciplinary plan of care for residents, from admission to discharge. You will act as a liaison between resident/family and healthcare personnel to ensure necessary care is provided promptly and effectively. The Social Services Designee coordinates the overall psychosocial care plan for residents, provides support, and advocates for residents/patients and their families. Additionally, you will develop and maintain community resources for residents.
Requirements
- Related coursework in social work or psychology
- Hospital or nursing home experience, preferred
- Able to read, write, and communicate in English

Pay
$30 to $36 per hour.
