About the role
We are seeking a dependable, compassionate, and organized Social Services Designee to support the emotional, social, and psychosocial needs of residents in our long-term care and rehabilitation facility specializing in geriatric care. This position plays an important role in working closely with residents, families, and interdisciplinary team members to help ensure quality of life throughout their stay.
Responsibilities
- Assist in planning, organizing, implementing, and evaluating the facility’s Social Services program.
- Serve as an advocate and liaison for residents and their families regarding social, emotional, and psychosocial needs.
- Assist residents and families with adjustment to the facility and changes in condition or level of care.
- Identify and address medically related social and emotional needs of residents on an individual basis.
- Communicate and collaborate with nursing, therapy, administration, and other members of the interdisciplinary care team.
- Maintain accurate and timely documentation in accordance with facility policies and applicable regulations.
- Assist with care planning and discharge planning as appropriate.
- Help connect residents and families with available community resources and services when needed.
- Ensure Social Services practices remain in compliance with applicable federal, state, and local regulations.
- Perform additional Social Services duties as directed by the Administrator and Social Services Director.
- Complete all required resident assessments, screenings, and social services documentation accurately and within required federal, state, and facility timeframes.
Requirements
- Education in Social Sciences, Social Work, or a related field preferred.
- Long-Term Care or Healthcare Experience preferred.
- Strong Documentation and Organizational Skills.
- Excellent Communication and Follow-Up Skills.
- Ability to Work Effectively with Residents, Families, Staff, and Community Resources.
- Dependable, Compassionate, and Detail-Oriented.
Licensed Social Worker in Alabama NOT required.
