Social Services Designee

What You’ll Do Support residents through psychosocial assessments and individualized care planningHelp residents and families navigate the transition to long-term and memory careComplete MDS-related documentation and maintain accurate, compliant recordsAdvocate for resident rights, dignity, and self-determinationCoordinate discharge planning and community resourcesCollaborate with nursing, administration, and the interdisciplinary teamAssist with survey readiness and regulatory compliance What We’re Looking For Certified, licensed, or registered in the state of practice (required)Bachelor’s degree in Social Work or a related Human Services fieldAt least 1 year of social work experience in a healthcare or geriatric settingSkilled Nursing Facility experience preferredNursing experience in a SNF a plus Why Monarch Springs?
