Responsibilities

• Aid in preparing residents for their future
• Ensure residents' social and emotional needs are met
• Congnorate resident discharges
• Participate in resident care conferences
• Ensure residents are comfortable and safe

Requirements

• Prior Social Services Designee experience in a Skilled Nursing Facility
• Bachelor’s degree in Social work or similar field (preferable)
• Five (5) years of related experience (preferable)
