About the Role
Orchard Grove is a 130-bed facility specializing in Short-Term Rehabilitation (STR), Long-Term Care, and hosts a dedicated secured memory unit. Our team includes APRNs, with rehabilitation staff trained in the specialized LSVT Big and Loud program, catering to individuals with Parkinson's and Parkinson-like diseases. Our staff boasts remarkable longevity and actively engages within the community, each bringing unique talents to enrich our patients' lives.
Position: 40 hours per week. Reports To: Director of Social Services.
Responsibilities
- Craft and document personalized care plans for residents' social and emotional well-being in the electronic medical record.
- Collaborate with nursing, MDS, Admissions, and rehabilitation departments to facilitate effective discharge plans for residents.
- Know and comply with the Residents' Bill of Rights; inform and educate staff, residents, families, and responsible parties of personal, property, and health service rights.
- Maintain updated resources for residents and families regarding community resources, services, agencies, and organizations.
- Maintain a record of all reported concerns, assist in resolving them, and assure appropriate follow-up and documentation.
- Remain easily accessible to residents, families, and staff for support and guidance.
- Provide timely referrals to appropriate agencies for assistance (e.g., conservatorship application, Medicaid application).
Requirements
- Bachelor's Degree from an accredited program in Social Work or a related field.
- Knowledge of admissions and discharge procedures in conjunction with all current laws.
- Familiarity with residents' rights and documentation requirements in accordance with current regulations and/or guidelines.
- Attention to detail, good follow-through skills, and ability to prioritize multiple tasks.
- Innovative and able to find creative ways to serve a variety of resident needs while maintaining customer focus.
- Must have a vehicle and a valid driver's license.
- Demonstrates cooperation in assisting others and ability to work as a team.
- Patience, tact, enthusiasm, and a positive attitude toward residents and their support networks.
- Ability to remain calm in stressful situations.
- Ability to communicate with residents, staff, and family members in a professional manner.
