Social Services Designee

About the role

We are currently searching for a Social Services Designee to join our friendly, caring and supportive team at Harmony Cedar Rapids. Responsibilities

• Assist in preparing our residents for their future
• Ensure our residents receive the high standard of care they have grown to expect by ensuring our residents' social and emotional needs are met during their stay
• Coordinate resident discharges
• Participate in resident care conferences
• Ensure residents are comfortable and safe

Qualifications

• As a minimum, a high school diploma or equivalent
• Two (2) years of experience in a Social Worker role, preferably in a healthcare setting

Preferred qualifications

• Prior Social Services Designee experience in a Skilled Nursing Facility
• Bachelor's degree in Social Work or similar field
• Five (5) years of related experience
