Responsibilities
- Develop a comprehensive social history and assessment of the resident that includes problems, strengths, special needs, preferences (social, religious, and/or cultural), and implications for the care plan.
- Assist with the coordination of Health Center admissions and assessments.
- Help residents and families transition to the new environment, orient them to the facility, services, limitations, and resident’s rights.
- Participate in the development of individualized care plans, including goals and approaches for the Social Services component.
- Attend scheduled care planning conferences, addressing issues of schedule, treatment, and environment, and encourage resident and family participation.
- Ensure Advanced Directives are complete, maintained, and placed in resident charts.
- Complete all mandatory documentation in a timely manner as required by regulations.
- Maintain confidentiality of admission information and resident records in compliance with HIPAA and the community’s Code of Ethics.
- Maintain regular, ongoing relationships with families to discuss needs or concerns and mediate issues between residents, families, and staff.
- Coordinate discharges and transfers to ensure appropriateness and continuity of care, developing comprehensive discharge plans with residents, families, and multidisciplinary staff.
- Provide or identify referral, agency, and community resources to meet resident and family needs.
- Coordinate support groups, participate in policy decisions affecting resident life, and advocate for Residents’ Rights.
Requirements
- Bachelor’s degree in Social Work or Human Service field.
- At least 1 year of supervised work experience in a health care setting, working directly with elderly individuals, residents, and families.
- Skilled Nursing experience (required).
- Discharge planning experience.
- Home health provider connections.
- Strong documentation skills.
- Understanding of DCF guidelines and reporting.
- Managed care and Medicare guideline knowledge.
- PDPM and care plan knowledge.
- Excellent customer service, communication, and organizational skills to manage a large volume of patients.
