Social Services Case Manager

This role requires critical thinking, effective communication, and decisive judgment to build positive relationships and lead others when necessary. Key Responsibilities Include Providing mental health assessments and support within designated areas such as Dialysis, Transplant, Cystic Fibrosis, and the Emergency Department.Applying licensed clinical social work skills to evaluate and address patient needs.Collaborating with healthcare teams to deliver patient- and family-centered care. Qualifications And Skills Minimum of one year of relevant work experience.Preferred licensure as a Licensed Clinical Social Worker (LCSW) in Florida.Background in psychology, mental health, marriage and family, or related social services fields.
