Social Services Case Manager

About the role

Responsibilities

• Providing social work services in specialized areas such as Dialysis, Transplant, Cystic Fibrosis, and mental status exams in the Emergency Department, with a focus on patient-centered care.

Qualifications and Skills

• At least one year of relevant experience
• Licensed Clinical Social Worker (preferably in Florida)
• Educational background in Psychology, Mental Health, Marriage and Family, or related social services fields
