General Description

Under general supervision, the worker performs a variety of health and human service program activities in a helper capacity, involving extensive client contact. The worker often works independently in consultation with a professional staff member. Certain assignments may require specific training prior to or within a set period after employment. The role requires the use of an automobile for travel and may involve potentially dangerous situations for both client and worker. Mandatory overtime may be required.

  • Data entry in the FACTS computer system
  • Set up and record multidisciplinary treatment meetings
  • Schedule appointments with appropriate medical personnel, including physical and mental health providers

Minimum Qualifications

  • Education: High school diploma or equivalent
  • Experience: None required
  • Special Requirements: Valid driver’s license
