About the role
Join our Social Services team as a full-time Social Services Representative and serve as an advocate for geriatric patients and their families. This position involves working in a healthcare setting that values teamwork and offers significant growth opportunities.
Responsibilities
- Work with individual patients and families to help them adjust to center life and cope with crises related to illness, disability, and death.
- Serve in an advocacy role for patients and their families, assisting them in expressing, defining, and resolving grievances.
- Coordinate discharge planning functions.
- Report the current status of social services and present needs to the Director of Social Services.
- Participate in developing patient care plans in cooperation with professionals from other disciplines.
- Maintain confidentiality of professional information.
Requirements
- Bachelor's degree from an accredited college or university.
- Knowledge of the medical case management process, needs of geriatric patients, healthcare settings, and funding resources such as Medicare Part A, B, C, and D, Medicaid, Managed Care, and other private insurance.
- Imagination and creative ability, skill in working cooperatively with other professionals, and the ability to organize and carry out responsibilities efficiently and effectively.
Insurance authorization experience is preferred.
