Social Services Assistant

Responsibilities

• Support the social service function in a long-term care and short-term rehabilitation nursing center.
• Work closely with the Social Services Manager to welcome and admit new residents and patients.
• Complete documents related to psychosocial assessments, care plans, and discharge plans.
• Ensure a comfortable and overall positive experience is achieved while completing treatment plans.

Qualifications

• A High School Diploma or equivalent is required.
• Post-secondary education or work experience in a social service or health-related field is preferred (such as social services, social work, patient registration, care coordination, or discharge planning).
• Must be familiar with Medical Terminology.
• Must be proficient in the use of personal computers (Windows, Internet, and Applications/Programs navigation).
