Social Services Assistant

About the role

Support the social service function in a long-term care and short-term rehabilitation nursing center. Work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related to psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience while completing treatment plans.

Schedule

Monday – Friday, dayshift.

Requirements

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years’ experience in health care education, marketing, insurance, or commensurate education preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.
