About the role

The primary purpose of this position is to provide services that respond to the emotional needs of residents and their families, assist with resident admissions, and support the referral process.

Responsibilities

• Assess and address the emotional needs of residents and their families
• Assist with resident admissions and the referral process
• Coordinate the discharge planning process

Requirements

• One (1) year experience in long-term care is preferred
• Ability to assess resident needs and coordinate discharge planning
• Valid driver’s license
