Responsibilities
- Affirms that social service/discharge planning activities are integrated and coordinated within the hospital system through effective communication at all levels.
- Develops specific, measurable, realistic plans and objectives to enhance social service delivery.
- Performs psychosocial assessments and reassessments on all residents.
- Ensures resident safety and reduces the likelihood of medical/health care errors by optimizing the environment of care.
- Interprets and promotes residents' rights and the "Residents' Bill of Rights" with the Administrator.
- Interacts professionally with resident/family members and involves them in the formation of the plan of care.
- Coordinates and directs Social Service/Discharge Planning Department activities to ensure resident needs are met and facility policies are followed.
- Collaborates with other departments to improve resident care and performance.
- Plans, assesses, coordinates, and determines the services needed by prospective residents for admission to the facility.
- Maximizes facility census, implements an effective admissions process, and ensures high resident satisfaction.
- Conducts tours of the facility for prospective residents/families.
- Receives resident referrals and tracks pre-admission referral activity daily.
- Assists with admissions arrangements and confirmations, screens potential admissions, and ensures proper paperwork completion.
- Expedites the assessment and admissions process in an assertive manner.
- Maintains working relationships with referring community agencies.
- Promotes the facility’s public relations within the outside community and other healthcare settings.
Qualifications
- Bachelor's degree in social work is preferred.
- Current state clinical social work licensure is required.
- Short-term rehab experience is preferred.
- Two years of experience in Social Services background or a behavioral setting is preferred.
- Candidates must maintain a working email address and phone number for employer communication.
- Able to communicate effectively in English, both verbally and in writing.
- Bilingual preferred.
- Basic computer skills/knowledge.
