Social Services Assistant

Wage range: USD $17.00 - USD $18.00 per hour, based on skill sets, experience, education, licensure, and organizational needs.

Responsibilities

• Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission.
• Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident.
• Work closely with Case Management to coordinate patient discharges.

Requirements

• High School Diploma or equivalent G.E.D (minimum requirement).
• Social Work License preferred, but not required.
• Experience in long-term care or skilled nursing facility.
• Reliable transportation required.
