Social Services Manager Responsibilities
- Assess residents’ psychosocial, emotional, and discharge planning needs
- Develop and implement individualized social service interventions
- Cookordinate discharge planning and transitional care services
- Assist residents and families with care planning and community resources
- Facilitate advance directive discussions and decision-making
- Cookordinate Family Council meetings and family support initiatives
- Manage resident grievance and complaint processes
- Ensure compliance with state, federal, and company regulations
- Cookoperate with interdisciplinary healthcare teams to enhance resident quality of life
- Advocate for resident rights, dignity, and emotional well-being
Required Qualifications
- Bachelor’s Degree in: Social Work (BSW), Human Services, Psychology, Sociology, Gerontology, Rehabilitation Counseling, Special Education or related field
- Minimum one (1) year of supervised social services experience in a healthcare setting
- Experience working with elderly residents or geriatric populations
- Strong communication, documentation, and organizational skills
Preferred Qualifications
- Licensed Social Worker preferred (LBSW, LSW, LMSW, LCSW, etc.)
- Prior Skilled Nursing Facility (SNF), Rehabilitation, Long-Term Care, Nursing Home, or Healthcare Social Services experience strongly preferred
