About the role

A Care Coordinator is a trained healthcare professional who exhibits superb communication skills. They work with patients to determine needs and evaluate interventions. Care coordinators possess leadership qualities needed to guide individuals of varying backgrounds toward a common goal for the betterment of a patient’s health and quality of life.

Responsibilities

  • Developing and coordinating health care programs
  • Communicating between staff, patients, and family
  • Handling patient case management and education
  • Recruiting and training staff and creating schedules
  • Developing patient goals and monitoring progress
  • Maintaining patient confidentiality and quality care

Qualifications

  • Prior experience working with patients in either an administrative or clinical role required
  • Bachelor’s Degree (BA), Licensed Master’s Social Worker (LMSW)/Licensed Clinical Social Worker (LCSW)/Licensed Marriage and Family Therapist (LMHC), Registered Nurse (RN)/Licensed Practical Nurse (LPN) degree
  • 2 years minimum case management experience
  • Strong understanding of cultural competency with the target population
  • Computer literacy required
  • Interest in working with HIV-positive clients
  • Exposure to issues of death and dying

Schedule

  • All positions are in hospitals and nursing homes
  • These facilities are open 24 hours/7 days each week
  • General shift availability: Days (7a-3p or 8a-4p), Evenings (3p-11p or 4p-12a), Nights (11p-7a or 12a-8a)
  • Every other weekend is usually required for all positions
