- Assist residents and their families with the facility admission and orientation process.
- Help coordinate safe, comprehensive discharge planning and community resource referrals.
- Maintain accurate, up-to-date resident medical records and social service documentation.
- Conduct regular room visits to assess residents' emotional and psychosocial well-being.
- Advocate for resident rights and promptly report any grievances to the Director.
- Assist in organizing resident council meetings and family support initiatives.
