Social Service Assistant

Responsibilities

• Execute a comprehensive social services program, offering counseling and support services to residents and families
• Assess and address residents' medical-related social needs
• Coordinate family meetings and facilitate ongoing discussions between residents, families, and the treatment team
• Advocate for residents' rights while liaising between all parties involved
• Manage discharge planning efficiently
• Ensure residents and families receive top-notch service in a compassionate atmosphere that respects individual needs and rights

Requirements

• Prior experience in long-term care is a plus, but not required
• Experience coordinating social services for the senior population is required

Schedule

Part-time position, 25 hours per week, day shift Monday‑Friday 7 am‑2 pm, no weekends.
